A not too uncommon occurrence with progressive loaders. I used to have 5 or 6 a year come in, for me to try to see what happened.
That's why I've always done all my loading one at a time, on a 1969 vintage RCBS Rockchuker.
I prepare the brass and put in the primers.
Then when I picked up a primed case, I don't lay it down until its finished loaded.
I also tend to use powders that fill up the case enough that a double charge won't fit.

Look right above the pictures where it says "Read the Story" in big blue letters.
